Delta Dental PPO (Standard) FAQs
Do I need to visit a specific dentist?
No, you may visit any licensed dentist; however, you may save the most in out-of-pocket costs by visiting a Delta Dental PPO™ dentist.
Where can I find a dentist (or see if my dentist participates)?
You can find participating dentists by visiting the Find a Dentist page or by calling Delta Dental’s customer service department at 844-223-8520.
Does everyone in my family have to use the same provider?
No, each member of your family can see a different provider.
How will my dentist receive payment?
For Delta Dental PPO and Delta Dental Premier® providers- Payment will be sent directly to your dentist. For nonparticipating providers- You will be responsible for making full payment to your dentist and then Delta Dental will send you the check for covered services.
What if I am in the middle of orthodontic treatment, crown, bridge, dentures etc.?
If orthodontic treatment is currently in progress for you or one of your dependents, please ask your dentist to submit a new treatment plan to Delta Dental. The remaining liability of the claim will be recalculated based on the number of months left in the treatment plan. Delta Dental will also receive the orthodontic lifetime maximum history from your previous carrier. Your orthodontic maximum benefit available under your new plan will be reduced by the benefit amount used under your previous plan.
Will I get an ID card?
Yes, Delta Dental of Michigan will provide two ID cards with the enrollee’s name.
Do I need an ID card to receive treatment?
No, simply tell your dentist that you have Delta Dental and they will be able to contact us for your benefit information.
What if I have a dental emergency?
Most dental offices have an after-hours emergency number you can call.
Should I tell my dentist my coverage changed?
Yes! Please tell your dentist that Delta Dental of Michigan is providing you with coverage.
Where can I find information about my plan and coverage levels?
Review the Delta Dental Benefit Summary for Ford Salaried Active Employees.
Do I need a referral to see a specialist?
No, you do not need a referral to see a specialist.
My spouse has dental insurance can I use both and how will that work?
Yes, Delta Dental will coordinate with another carrier. If you and your spouse are both eligible to enroll in this plan, you may be enrolled together on one application or separately on individual applications, but not both. Your dependent children may only be enrolled on one application. Delta Dental will not coordinate benefits between your coverage and your spouse's coverage if you and your spouse are both covered as enrollees under this plan. If your spouse is the primary enrollee under a different dental plan, then this plan would process as secondary. When Delta Dental is the secondary plan, it will pay for covered services based on the amount left after the primary plan has paid. It will not pay more than that amount, and it will not pay more than it would have paid as the primary plan.
